By submitting this form, the participant(s) (or their parent/guardian, if under 18) acknowledges the physical and inherent risks involved in participating in the 2nd US Open Pencak Silat Championships, to be held October 10–12, 2025, in Herndon, Virginia. This includes, but is not limited to, risks of physical contact, accidental injury, emotional stress, and travel-related incidents. In consideration of being allowed to participate, the participant hereby voluntarily and irrevocably agrees to release and hold harmless the USA Pencak Silat Federation, the Silat Institute, Abdul-Malik Ahmad, event staff, volunteers, venue hosts, and all affiliated parties from any and all claims, demands, actions, or causes of action arising out of or related to any injury, loss, damage, or death sustained while participating in or traveling to/from the event.
