By submitting this form, the participant (or their parent/guardian, if under 18) acknowledges the physical and inherent risks involved in participating in the 2nd US Open Pencak Silat Championships, to be held October 10–12, 2025, in Herndon, Virginia. This includes, but is not limited to, risks of physical contact, accidental injury, emotional stress, and travel-related incidents.

In consideration of being allowed to participate, the participant hereby voluntarily and irrevocably agrees to release and hold harmless the USA Pencak Silat Federation, the Silat Institute, Abdul-Malik Ahmad, event staff, volunteers, venue hosts, and all affiliated parties from any and all claims, demands, actions, or causes of action arising out of or related to any injury, loss, damage, or death sustained while participating in or traveling to/from the event.

Participants under 18 must have this form signed by a parent or legal guardian. All participants are required to have completed a physical exam by a licensed medical professional within the last year, confirming their fitness to participate in martial arts activities. Should any legal liability be found against the Silat Institute or its affiliates, any financial compensation will be limited to the lesser of actual documented medical costs or $2,000.

All registration fees are non-refundable under any circumstances. Additionally, the participant and their guardian consent to the use of photographs, video footage, interviews, and other media taken during the US Open for promotional and archival purposes by the organizers on platforms such as YouTube, Facebook, Instagram, and future marketing materials, without expectation of compensation.

This waiver is a required condition of participation.
